When an employee of the Sioux City Community School District sustains an injury, it not only impacts that employee’s life, but the injury also impacts his or her family, and the continuity of the students we educate.
Workers’ Compensation Injury Procedure
A. All Near Misses – Declination of Medical Treatment
OSHA defines a Near-Miss as an unexplained event that did not result in injury, illness, or damage – but had the potential to do so. Only a fortunate break in the chain of events prevented an injury, fatality, or damage.
Notify Human Resources of the Near Miss, within 24 hours of the Near Miss.
Human Resources will provide required paperwork, complete the required documents within 24 hours.

B. All Injuries Needing Medical Treatment:
In the event of a severe injury, call 911. Otherwise, notify your supervisor or principal of your injury/incident.
For all injuries, call Company Nurse at 1 (888) 375-0284. Give them your employer's name of Sioux City Community School District, with search code QT837. Please keep a record of your incident/claim number.
Seek medical attention as directed by Company Nurse. You may be referred to self-care, an occupational health practitioner, urgent care, or to go to an emergency room.
After your medical visit, you will be required to complete the necessary paperwork. Human Resources (HR) will be notified of the incident by Company Nurse and will provide you with the required paperwork. Please submit the paperwork within 24 hours to your principal, supervisor, and to HR. For assistance, or any concerns, call human resources at (712) 279-6692. Attend any follow-up appointments as directed by the workers’ comp physician or nurse’s care plan until the injury/illness is resolved.
Submit all duty restrictions and return to work documents to your principal, supervisor, and the HR department before your next work shift. For light duty restrictions, complete the temporary modified/light duty form with your principal or supervisor.
Note: If you choose to be treated by a personal doctor, you may not qualify for workers’ compensation insurance benefits and you may be responsible for all medical costs related to this incident. This is in accordance with Iowa Workers’ Compensation statute, Iowa Code Chapter 85.27.
Exposure Control Plan for Bloodborne Pathogen
It is our goal to reduce the significant risk of infection by minimizing or eliminating occupational District exposure to blood and other infectious materials by educating about universal bloodborne pathogen precautions.
